If you are interested in using your insurance to cover your appointments you will need to contact your insurance to determine your benefits. There is a form provided for your convenience on the Patient Forms Page. You are required to pay any deductible or co-pay amounts at the time of your appointment. If you wish to use insurance but do not contact your insurance prior to your first appointment you will be required to pay the entire amount of the cost of the appointment. Any missed appointments or appointments cancelled less than 24 hours in advance will be your responsibility. Insurance does not cover these charges.
PAYMENT ASSISTANCE
If you wish to have your bishop or other clergy help pay for your visits you need to provide a letter from them stating they are accepting financial responsibility for your visits. This letter should include contact information (mailing address and phone number). Any missed appointments or appointments cancelled less than 24 hours in advance will be the patient responsibility.
WHAT TO EXPECT
EVALUATIONS - During this visit we will go over a great deal of information including: - current/past medications including supplements, herbal products, and over-the-counter products - current/past symptoms including sleep, appetite, energy, motivation, focus, etc. - your medical history including a body systems review family psychiatric and medical history for blood relatives including siblings, parents, aunts and uncles, and grandparents.
The initial evaluation takes about two hours. This allows time for us to cover all relevant information helping to ensure an accurate diagnosis. We will also discuss treatment options which may include supplements, medication, therapy, recommendations for dietary changes, exercise, and sleep hygiene.
Once a plan of action has been agreed upon education will be provided regarding expected benefits of treatment, duration of treatment, dosing, and side effects. If a new medication is started you will be expected to return for a follow-up appointment within two to three weeks of your initial evaluation. You must keep your medication follow-up appointments in order to receive additional refills. Most patients will have two to three follow-up appointments during the first three months of treatment. Once treatment goals have been achieved patients are required to be seen once every three months for a medication follow-up appointment and to receive medication refills.
Some people wonder why it is necessary to be seen every three months and why they cannot have refills for a longer period of time. Over a three month period there are things which may occur that may alter the effectiveness of your treatment. These might include: changes in health, new medical problems, medication changes from other providers, and new supplements or herbal products. In addition, there may be new symptoms that you wish to discuss and treat.
THERAPY - Therapy sessions last between 45-50 minutes unless you have scheduled an extended visit. In order for therapy to be effective it is necessary for you, as the patient, to come prepared to talk and discuss any concerns, problems or challenges you may be having. It will be up to you to decide how often you will come in for therapy. It is recommended that sessions are a minimum of every two weeks at least initially. This frequency provides us time to develop an open rapport, define your treatment goals, and decide on a plan of care.
Therapy sessions may consist of activities such as learning new skills, discussing and analyzing relationship interactions, and education about disorders and/or medications. Therapy may be conducted on a structured schedule with pre-planned topics or it may follow a more natural flow and address topics that are of greatest concern to you at the time of your visit.
If you are interested in couples or family therapy and would like to have individual time as well it may be possible for us to schedule back to back appointments. This would allow time for individual needs to be addressed and some shared time for couple or family concerns.
MEDICATION MANAGEMENT - Medication management appointments are 20-25 minutes. During this time we will discuss how well the medication(s) is working, side effects, any new concerns or symptoms, and additional education is provided if necessary. Refills will be provided during your appointment. If you miss your scheduled medication management appointment you will be required to re-schedule in order to receive additional refills. Refills are not called out to pharmacies unless there are extenuating circumstances that caused you to miss your appointment.
If you are attending therapy sessions in addition to receiving medication management you may choose to combine your appointments. There is no additional charge to receive refills for maintenance medication during a therapy session. You will be billed the additional charge for a combined appointment when there are changes made to your current medication plan or additional medication education is provided during the appointment.
