Shauna Norton, Psychiatric APRN, is currently on the Blue Cross Blue Shield of Utah panel and the IHC - Select Health panels. If Shauna Norton, Psychiatric APRN, is not a preferred provider on your insurance you will be responsible to pay for your appointment at the time of service. You will be provided with a reciept or cpoy of your bill for you to send to your insurance company for reimbursement.
Using Insurance Benefits
Services may be covered in full or in part by your health insurance or employee benefit plan. It is your responsibility to contact your insurance before your appointment to determine your out-of-pocket cost. Your out-of-pocket cost is due at the time of your appointment.
Insurance Benefit Verification
Please print and use the INSURANCE BENEFITS VERIFICATION FORM when you contact your insurance company. This form is provided for your convenience to help you determine what your insurance benefits are for outpatient mental health services. It is necessary to have your insurance benefits verified prior to scheduling your first appointment. This will help you determine your total out-of-pocket costs for your first visit.
Some of the questions you will need to ask your insurance company are listed below. The easy to use form has all the questions you will need to ask your insurance representative.

- Do I have a yearly deductible for outpatient mental health? If yes, how much of it has been met this year?
- What is my co-pay for outpatient mental health, after my yearly deductible has been met?
- Does the calendar year for my coverage start in January or some other month?
- How many sessions/visits per calendar year does my plan cover? How many have been used this year?
- Does insurance send payment to my provider or am I reimbursed for charges submitted?
- Do I need to pre-authorize my visits? How many visits are authorized? How many have been used this year?
- Do I need a referral from my primary care physician?
Payment
Payment is due at time of service and includes any deductible or co-pay amount your insurance does not cover. Cash, check, debit and credit cards (Visa, Mastercard) are accepted for payment. Yearly deductible amounts will need to be paid at the beginning of each calendar year before your insurance will cover the cost of your appointments.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the appointment as scheduled. Please be aware that insurance does not pay for missed appointments. You will be responsible for payment in full.
