Psychiatric Medication Evaluations are provided for The Juvenile Court System by referral only. Once the referral is sent to my office you will be contacted to schedule the evaluation. There is no cost to the family for the evaluation. 

There will be a few assessment forms in the waiting area that will need to be filled out by the adolescent. While the adolescent is filling these forms out, time is spent with the parent(s) or guardian(s) to discuss their concerns or observations and to go over family history and the adolescents medical history. The remainder of the time is spent with privately with the adolescent (unless they would prefer to have one or both parents present.)

At the end of the appointment I will meet with the adolescent and the parent(s) or guardian(s) to explain my recommendations. The appointment is for the evaluation only. If medication is recommended we will discuss options for treatment. You are not required by the court to see me for medication management. You may see your family medical provider or another prescriber for medication management.
 

If you will not be able to keep your appointment for any reason please contact my office as soon as possible so that I may offer that evaluation time to other patients who are waiting to be seen.

If you do not show up for your appointment or you cancel your appointment less than 24 hours in advance (unless it is an emergency) your Probation Officer will be notified.
